Application procedures for Certificate of Doctor of International Chinese Medicine

1. Application qualification:

• Applicant must have a medical license, or an oriental medical license, or dentist license, or acupuncturist license from U.S. or a foreign equivalent.
• Applicant must have at least associated bachelor degree, or qualified result from International Chinese Medical Examination.

2. Application materials:

• Medical License or Certification of International Chinese Medical Exam (English translation required)
• One colored Passport copy
• A completed application
• Four current 2” x 2” photos
• US $200 application fee, US$1,300 tuition fee, US$300 examination fee
• US$1,200 certification fee

3. Authorization:

Authorization to all directors to accept application.
